Ken Kaneki, an eighteen-year-old first-year student at the Faculty of Philology, is the protagonist. He became half-ghoul as a result of organ transplantation. While the panel had previously been used as a profile photo, its link with emotions of agony and self-pity did not become widely recognized until late August 2019, with the developing meme popularity of Tokyo Ghoul anime theme song UNRAVEL, commonly known as "Donny's Theme," playing a big role.

During the Tsukiyama Family Extermination Operation, Sasaki had a recollection from his childhood as Ken Kaneki in which his mother physically assaulted him. He also understood that he had subconsciously desired to die and had been relieved to meet Arima during the Owl Suppression Operation. However, it did not work, so he decided that burying his history would be his salvation, so he opted to forget and live as Haise Sasaki. Sasaki embraced his history after discovering this and resolved to cease "dreaming." In stark contrast to his formerly mild disposition, Sasaki grew colder and much more aggressive after retrieving his memories. Despite recalling his old friend, he battled with more ferocity and was prepared to endanger Tsukiyama's life. He did, however, make certain that Kanae would save Tsukiyama before hurling the ghoul from the building. When Urie blamed him for Shirazu's death, he calmly told him that it was his responsibility since he was fighting with Shirazu. He then wept alongside Mutsuki and Saiko, demonstrating that, despite his harsh attitude, he was saddened by Shirazu's death.
